TERMS and CONDITIONS

USDR Lab Services are offered by U.S. Doctor Resources, L.L.C. a registered U.S. Company doing business in Michigan and Indiana. Due to State Law in these five states, we cannot provide laboratory testing in New York, New Jersey, Massachusetts, Maryland or Rhode Island. To request laboratory services in these states, you must have a signed requisition from a physician registered in the respective state. Quest™ Diagnostic will refuse to perform any and all Laboratory Services requisitions processed on line in these states.

I have read and agree to the TERMS AND CONDITIONS.

If I am experiencing symptoms or have concerns, I recognize that I must see a healthcare professional as soon as possible.

I understand that these laboratory tests are in addition to healthcare visits with a physician and do not replace the evaluation and testing by healthcare professionals. I will send copies of my laboratory tests to my physician for evaluation and storage within the physician’s/ healthcare professional’s medical records. I will complete the physician’s name, address and contact information on the Order Form.

The Terms and Conditions include a separate consent for HIV testing and HPV/STD/Pap smear testing, and the Privacy and Security Policy. It is acknowledged and agreed that for privacy protection Quest™ Diagnostic Laboratories will only forward the laboratory results via a secure link to the individual and his/her physician by email or mail address as entered during checkout. Quest™ Diagnostic Laboratory will follow the state and federal laws as to reporting of certain communicable diseases. Should the individual being tested elect to share any laboratory results, it will be up to him or her to do so directly.

LOCATION

Lab Services are offered online as a business service through our office based in Indiana. They are offered to residents of the United States where they do not conflict with state law which at this time includes NY, MA, MD, NJ, RI. By using this online service, you agree to be bound by the following terms and conditions. USDR Lab Services reserves the right to make changes including pricing to the terms and conditions at any time. Changes will be effective immediately upon posting of the modified terms and conditions. Permission to use this site is at the discretion of USDR Lab Services and may be terminated at any time for any reason.
  1. You agree that you are over the age of 18.
  2. You agree that the information you submit during the course of purchasing a laboratory test from USDR Lab Services is truthful and accurate.
  3. You agree not to copy any images, text, or structure from this website. You also agree not to use the website for illegal purposes.
  4. You agree to use the site according to the instructions and intended use and not to disclose your password to any other users.
  5. You agree to abide by the purchase and understand that there are no refund policies of USDR Lab Services. No refunds are permitted as the requisition for Quest™ Diagnostic Laboratories services is processed immediately.
  6. You agree that a laboratory test purchased through USDR Lab Services does not constitute medical advice and will not diagnose, treat, or cure any disease or condition. You also agree that the laboratory test results can be inaccurate through no fault of USDR Lab Services. You understand that the service provided by USDR Lab Services is a service provided at your request and not suggested by our staff or staff physicians. As an online service we do not have a traditional doctor-patient relationship and only a physician/healthcare provider can provide CPT codes, diagnosis codes or provider information in order to file insurance claims. This information could be made available to you should you review this request with your establish physician with whom you have a patient relationship.
  7. The materials on the site are provided "as is" without any express or implied warranty of any kind including warranties of merchantability, no infringement of intellectual property or fitness for a particular purpose. USDR Lab Services offers no assurance of uninterrupted or error free service. USDR Lab Services does not warrant the accuracy or completeness of the information, text, graphics, links or other items contained on the site. Any of the information offered on the site may change at any time without notice.
  8. All displayed material on this website is for informational purposes only. USDR Lab Services makes no express or implied warranties, representations or endorsements whatsoever with regard to the services provided. USDR Lab Services shall not be liable for any cost or damage arising either directly or indirectly from any transactions conducted or information conveyed on the website. It is solely your responsibility to evaluate the accuracy, completeness and usefulness of all opinions, advice, services, merchandise and other information provided through the USDR Lab Services with your healthcare provider.
  9. Indemnification. You agree to indemnify, defend and hold harmless USDR Lab Services, its parent and affiliated companies, its officers, directors, employees, agents, licensors, suppliers and any third party information providers to the USDR Lab Service from and against all losses, expenses, damages and costs, including reasonable attorneys' fees, resulting from any violation of this Agreement by you.
  10. You agree to pay a posted administration/ processing fee in addition to the charges for each requested service in order to process the request. The administrative/processing fee is permitted under the state law where USDR Lab Services organized.
  11. Unused requisitions for orders will no longer be valid after 6 months from the original purchase date. Please note, requisitions that have not been used within 6 months from the date issued will expire, will be purged and will no longer valid for use.
  12. Discount coupon codes issued by USDR Lab Services are issued as a courtesy to our customers. Orders placed that do not meet the requirements or after expiration will be processed without the discount. Coupons are not redeemable for cash and cannot be applied when using in-house credits. Adjustments, credits and/or refunds will not be issued.
  13. Quest™ Diagnostic Laboratories will mail and/or fax copies of the laboratory test results copy to the individual and his/her physician whose names are listed on the order form. USDR Lab Services will not have access to this information. In the future, you may be able to store your complete Medical Records™ within a program underdevelopment which would retain your ordering history, products and laboratory test results.

I have read and agree to the Terms and Conditions for HIV Testing Consent (only applies when ordering an HIV test)

The purpose of this form is to document that I have requested that a blood test for the Human Immunodeficiency Virus (HIV) is to be performed. The blood test(s) are used to detect HIV antibody or DNA. HIV is the cause of Acquired Immune Deficiency Syndrome (AIDS). I understand that the test is performed by drawing blood from the arm and processing the resulting blood specimen by either ELISA, Western Blot or other laboratory technology. I have been informed that the tests can produce false positive and false negative results. For HIV, antibody testing, the ELISA screening test produces three false "positive" (indicating the presence of anti-HIV when it is not present) test results in every 10,000 specimens processed, regardless of population tested. Also, I have been informed that, if positive, the ELISA test will be repeated, and if still positive, a secondary level test (Western Blot) will then be performed. The combination of these two tests reduces the possibility of a false positive HIV antibody result to a very small fraction per 10,000 tests processed. I have been informed that the ELISA screening test fails to detect anti-HIV for a period of time immediately after infection with the virus. I have the opportunity of re-testing, if I so desire, if it is suspected that this has occurred. The ELISA screening test may fail to detect anti-HIV in rare cases. This has been explained to me in language that I understand: the nature of the blood test, it's expected benefits, it's risks, and alternative tests. The results of the HIV test will become part of my medical record by law. The confidentiality of my medical record will be maintained. However, I understand that my physicians or other health care provider may ask to see the results for medical reasons. I understand that positive test results must be reported to the appropriate State Health Division by Quest™ Diagnostic Laboratories. Confirmed HIV antibody test results will be made available (except as noted above) only in summary or statistical form in such a way as to preserve my confidentiality. I have read the previous information and I understand it. By checking the box on the order form, I acknowledge that I have been given all of the information that I have requested concerning the blood test(s) and the release of results. I acknowledge that I have given my consent for the performance of laboratory tests and/or an HIV blood test, and the release of the results as described above. I have read and agree to the Terms and Conditions for HPV Testing Consent (only applies when ordering a Pap smear) The purpose of this form is to document that I have requested that the following tests be performed: a cytology test for cervical disease (including Pap smear, and DNA test for the Human Papilloma Virus (HPV) and a DNA test for STDs (sexually transmitted diseases)) that can include chlamydia and gonorrhea. The cytology test(s) are used to detect HPV DNA, STD infections and/or changes from normal cervical cells. (HPV is thought to be the cause of cervical cancer). I understand that the test is performed by swabbing the upper vagina and cervix and immediately swirling the brush in the cytology fluid. The resulting cytological fluid specimen will be processed by DNA analysis and other laboratory technology. The medical literature suggests that more consistent results are an associated with genotyping for HPV 16 and HPV 18 DNA in addition to ThinPrep™ cytology.

I have been informed that the tests can produce false positive and false negative results. For HPV, STD and cytology screening, the tests may produce false "positive" (indicating the presence of HPV, STD or abnormal cervical cells when not present). Also, I have been informed that, if positive or abnormal, the HPV, STD and/or Pap test will need to be repeated by my physician, and treated. There is an additional level of standard testing or treatment, which includes vaginal and pelvic examination, cultures for STD and colposcopy and biopsy for abnormal cervical cytology. These tests will then need to be performed by the physician/ healthcare provider you have consulted. As a part of a national study, you are encouraged to complete and return the form that will need copies of your previous Pap smear, this Pap smear and the subsequent (next) Pap smear, cultures and treatments (healthcare professional’s notes or office visits). You understand that the reviewer will not and cannot recommend any direction of treatment: the records are sealed for retrospective analysis. All treatments remain entirely under the care of your primary physician with whom you should share the PAP smear results.

This has been explained to me in language that I understand: the nature of the tests, their expected benefits, its risks, and standard tests. I realize that these HPV, STD and cytology Pap smear testing are in addition to routine gynecologic examinations and does not replace healthcare services by licensed professionals.

I have read the previous information and I understand it. By checking the box on the order form, I acknowledge that I have been given all of the information that I have requested concerning the Pap test(s) and the follow up I must do to review the results with my physician.

I acknowledge that I have given my consent for the performance of HPV, STD and cytology Pap smear test, and the needed follow up thereafter.

Privacy and Security

Security Information collected and/or retrieved by the USDR Lab Services website is encrypted via a Secure Socket Layer (SSL) connection to protect your information from unauthorized parties. USDR Lab Services servers are located in a secure facility with strict security protocols in place to ensure your privacy is protected.

Medical Record Privacy

Only physicians and staff members of USDR Lab Services will have access to your personal health information (PHI). USDR Lab Services will not forward results to your physician or other agencies unless required by state law to do so. Quest™ Diagnostic Laboratories will forward copies of your laboratories tests to the individual at the address entered on the requisition order.

Other Information

USDR Lab Services collects anonymous information about visitors to our website. Information collected includes IP addresses, referring website, duration of stay, time/date, etc. This information is used for site statistics only and may be shared with third-parties for marketing purposes only.
