OBSERVATION:
Conversion Table: Dosage Range of Therapeutic Medications.
1.Prilosec/Omeprazole is an over-the-counter preparation with identical properties to the prescription medications; research shows they are equivalent when the dose is 40mg. OTC omeprazole is 20mg; so just double the dose for equivalent response.
2.There is a strong association with high dose and long-term use of these proton pump inhibitors and development of community acquired pneumonia. Practitioners need to be vigilant about adverse effects of PPIs and consider alternative therapies.
3. All PPI have a 50% treatment failure rate. The article reports "Each of the above [drug] treatments achieves transient symptomatic improvement only and follow-up of dyspeptic patients for 5-7 years showed the benign, but recurrent nature of the disease in 50% of the cases." The physiologic treatment of G.E.R.D. is related directly to the absence of stomach acid, therefor adding OTC Betaine HCl(2) and digestive enzymes(2) before meals may emerge as the simplest and safest treatment if tried first in cases without defined ulcers. It takes a pH in the stomach of 2 to close the esophagus, preventing reflex.
4.Not for use in children. Recent events and reports increasingly suggest misalignment between the interests of the pharmaceutical industry and those of public health. Johnson & Johnson illegally and effectively promoted Propulsid off-label for children despite internal company documents raising safety concern.
