Once the client chooses the desired service(s) from the individualized plan, in this Level II option we coordinate all aspects of care from admission planning to the client's placement or implementation of home services. We will then visit the client once service begins and follow up with another visit 1 week after services have been in place. In this way, we can monitor the quality of care and client satisfaction
